Integrated Listening Systems Process
The Integrated Listening System (ILS) Listening Protocol is an individualized approach and a unique program is created for each individual. Although the length of the listening program varies by individual, a typical program consists of 30 sessions (1 hour and 30 minutes each). These Integrated Listening System sessions are broken up into 3 Segments:
- First Segment (Receptive Phase)….15 days
- Second Segment (Active Phase)……8 days
- Third Segment (Active Phase)…….7 days
A 3-5 week break is given in between each segment. The entire program, including the 3-5 week breaks, is usually completed within a 3-4 month period.
We work with children as young as 24 months of age. We also work with expecting mothers so that the developing fetus can begin gaining the benefits very early in life from the listening treatment.
Although we are a pediatric therapy center and we primarily work with children under the age of 18, adults can also obtain the benefits of the listening program. There is no age ceiling for this treatment.
An initial listening consultation is given in order to determine the individual’s suitability for the listening program.
Step 1:
Listening Test and Evaluation
The Listening Test measures both air and bone conduction, and provides the basis for the individual’s program. The test takes about 20 minutes and is given at varying stages throughout the program to gauge progress and provide feedback to the therapist. An extensive interview and evaluation are done concurrent with the Listening Test to review relEJt history, concerns and goals. The administration of the Listening Test is dependent on the age, maturity level and cognitive functioning of the child.
Step 2:
Reviewing your auditory training program with your therapist
The specific program for each individual is determined after the interview/assessment and Listening Test analysis. Each individualized program outlines the length and number of listening sessions, combined with other interventions your therapist may be employing.
Step 3:
The Program
All programs vary according to the individual’s specific condition; however, a typical core program involves 30 sessions @ 1 hour and 30 minutes each, with two 3-4 week breaks in between. The phases (receptive and expressive) model the sequence of developmental language acquisition.
Phase 1 – The Receptive Phase
This phase combines full spectrum music with filtered sounds, primarily classical music, at gradually increasing frequencies. The receptive phase of the program “opens” the ear, charges the nervous system and improves auditory processing. The duration of this phase is generally 15 sessions @ 1 hour and 30 minutes each.
Three – Five Week Break allowing for assimilation
Phase 2 – The Receptive and Active Phase
Following the 3-5 week break, a Listening Test is given again. Changes/feedback is discussed and the program for the next 8 sessions is reviewed. the Mother’s Voice is inserted into the program in order to give the individual a sense of calm and security, similar to the feelings that one had in utero. This phase will target both the receptive and expressive areas.
Another Three – Five Week Break allowing for assimilation>
Phase 3 – The Active Phase
Final 7-session begins. The individual sings or reads into the microphone and the expressive phase addresses the ear-voice control loop, that is, the energizing effect and experience of hearing one’s own voice. The individual hums, sings, or speaks into a microphone; the sounds are electronically enriched and fed back through the headphones. The “expressive listening” increases our ability to try to listen to one’s self and move into effective dialog and communication with others.
Activities during Listening:
Listeners are free to walk about, play games, or just rest. Specific therapeutic activities designed by occupational therapists or other professionals may be done during listening. Children usually enjoy “quiet activities” such as crafts, playing with toys, painting, etc. while most adults prefer to sit and listen quietly. Homework, office work, and intense mental activities are discouraged. Chewing gum and eating are not allowed as they might interfere with the auditory stimulation.
Completion/Follow-up:
Upon completion of the program, a review of the initial goals is undertaken and progress is evaluated. A follow-up consultation with a therapist after 2-3 months is advisable to ensure that the gains are stable and are being maintained.
